Based in Stornoway, you will have an interest in working within all areas of Community Care.

Job Description

You will be required to complete professional assessments of need in conjunction with Health and other professionals, implement, manage and review agreed care plans.

Requirements

A Social Work qualification (Diploma/C.Q.S.W) and experience of assessment and care management is essential.

Full driving licence, registration with the SSSC and PVG Scheme Record will be required.

Appointment to the post will be for an initial period of three months.
