All salaries are inclusive of Islands Allowance. Appointment to the post will be on the first point of grade with annual incremental progression for permanent posts.

Job Description

Required for 21 hours per week, based at the Western Isles Hospital, to undertake assessment of need for patients prior to discharge from hospital. Appointment to the post will be for approximately six months to cover sickness absence period of the permanent postholder.

Requirements

You should be able to prioritise workload and work collaboratively with Home Care, Social Care, Health and Voluntary Agency colleagues to achieve high quality care interventions. 

Current driving licence is essential
